GetActive @ The Green 

GetActive @ The Green is a Short Breaks service for young people with complex needs and disabilities aged 12 - 18. Sessions run from 11 am to 3pm on Saturdays and focus on physical activities, inclusive sport and active play. Our weekly sessions are designed to help young people get out and about, socialise and engage with their community. 

As part of the programme we will work with you and your young person to train them how to travel to and from sessions. We pick young people up from their homes, travel to the session together, and return to your home afterwards. This helps them to gain confidence in travelling and building life skills.  

What Activities are on Offer? 

We plan each session to accommodate the preferences, interests and abilities of the young people. Most activities are held at our fully accessible Fortune Green Playcentre, with occasional trips to outside activities. Here are some of the most popular activities we offer:  

  • Football 
  • Basketball  
  • Parachute games 
  • Natural play 
  • Free play 
  • Water-based activities 
  • Nature walks 
  • Music and dance 
  • Yoga, Pilates, stretching 
  • Field trips in the community 

If your young person is interested in a particular activity, please let us know, and we will do our best to include it in the programme.  

Facilities

We hold most sessions at our secure and fully accessible Fortune Green Playcentre. Indoor facilities include a large activity area and sensory room. Outdoors, we offer a wide range of play areas including a garden, woodland areas, a multi-sports pitch and adventure playground. We occasionally visit Fairfield Playcentre, Camden Square Playcentre and WAC Arts, which are all secure, fully accessible and offer a range of extra facilities.

For Parents and Carers: How to Access Our Service

Places at our GetActive @ The Green service are allocated to eligible parents by Camden CYPDS (Children and Young People Disability Service) following a Short Breaks Panel meeting.

In order to apply for Short Breaks, you will need to fill in a a Self-Assessment Referral Tool (SART) Form (a professional such as a social worker, health professional or school SENCo can help you with this if required). You should then send the completed form alongside an annual review/clinical letter from a health professional or school to [email protected].  Once Camden Council has received your SART form, they will consider your eligibility for Short Breaks, this will include liaising with professionals stated on the form.

If you are already confirmed as eligible for Short Breaks and are interested in accessing our services, please speak to your allocated social worker or Camden Children and Young People with Disabilities Service Duty Officer.

What Happens When We Accept Your Referral?

We will meet with you and your young person to discuss your young person’s needs, the types of activities they enjoy and how we can best provide support. We will assess your transport needs at the point of referral and, if appropriate, offer your young person 1-to-1 travel training.
