What we offer Referrals Referrals Mentoring Any child or young person who maybe experiencing difficulties managing in school, pupil referral units, play/youth settings or the home environment may be referred to the mentoring project. Referrals may be made by education, safeguarding, health and other agencies working with children, young people and families. On receipt of an enquiry we will make an assessment as to whether the service can meet the needs of the individual child/young person. This assessment will include: Discussion with the young person Input from the family Input from other professionals Individual risk assessment Home visit The project will also consider any previously identified targets, objectives and behaviour plans. On completion of the assessment we will allocate a one to one keyworker who we feel will best be able to meet the needs and personality of the young person.
