GetActive @ The Green is a Short Breaks service for young people with complex needs and disabilities aged 12 to 18. Our weekly sessions focus on physical activities, inclusive sport and active play. They are designed to help young people get out and about, socialise and engage with their community.
As part of the programme, we will work with you and your young person to train them how to travel to and from sessions. We pick young people up from their homes, travel to the session together, and return home afterwards. This helps them to gain confidence in travelling and building life skills.
Session Times
11 am to 3 pm on Saturdays during term-time.
What activities are on offer?
We plan each session to accommodate the preferences, interests and abilities of the young people. Most activities are held at our fully accessible Fortune Green Playcentre, with occasional trips to outside activities. Here are some of the most popular activities we offer:

- Water-based activities
- Nature walks
- Music and dance
- Yoga, Pilates, stretching
- Field trips in the community
If your young person is interested in a particular activity, please let us know, and we will do our best to include it in the programme.
Facilities
We hold most sessions at our secure and fully accessible Fortune Green Playcentre.
Accessible
Our playcentre buildings are wheelchair accessible. We also have hoist changing facilities available.
Garden Area
We have an outdoor garden area where children can enjoy natural play, growing their own plants and even have messy fun in the mud kitchen!
Adventure Playground
We have a large outdoor adventure playground for children to enjoy running, climbing, playing and exploring.
Multi-sports Pitch
We have a floodlit multi-sports pitch so we can enjoy sports, including football, and outdoor games all year round.
Sensory Room
We have a calming sensory room to help children relax, regulate and recharge.
We occasionally visit Fairfield Playcentre, Camden Square Playcentre and WAC Arts, which are all secure, fully accessible and offer a range of extra facilities.

How to make a referral
Places at our GetActive @ The Green service are allocated to eligible parents by Camden’s Children and Young People Disability Service (CYPDS) following a Short Breaks Panel meeting.
To apply for Short Breaks, you will need to fill in a Self-Assessment Referral Tool (SART) Form (a professional such as a social worker, health professional or school SENCo can help you with this if required). You should then send the completed form, alongside an annual review/clinical letter from a health professional or school, to sart@camden.gov.uk. Once Camden Council has received your SART form, they will consider your eligibility for Short Breaks. This will include liaising with professionals stated on the form.
If you are already confirmed as eligible for Short Breaks and are interested in accessing our services, please speak to your allocated social worker or the Camden CYPDS Duty Officer.

If you are referred to our service, we will then discuss the needs of your young person with you and your referring social worker and arrange for the service to start.
We assign places at our GetActive Service through referrals from Camden’s Short Breaks Services.
Referring professionals can help parents complete a Self-Assessment Referral Tool (SART) Form and send the form alongside an existing report/annual review and a functional needs assessment to sart@camden.gov.uk.

Places will then be allocated at Short Breaks Panel based on the child or young person meeting the CYPDS eligibility criteria, an assessment being completed by a social worker/short breaks assessor, and an identified need being included within the Short Breaks Care Plan.
Once a space has been allocated at Short Breaks Panel, referring professionals should then contact Dominique Stewart at d.stewart@paceforall.com or call 07485 356 017 to discuss for referral and availability of space on the project.
Once this has been confirmed, please complete our referral form.
What happens when we accept your referral?
We will meet with you and your young person to discuss your young person’s needs, the types of activities they enjoy and how we can best provide support. We will assess your transport needs at the point of referral and, if appropriate, offer your young person 2-to-1 travel training.
